'Edit Transports' in mythtv-setup misses netid & tid setting

Description

When I try to add a DVB-C transport with the 'Edit Transports' option, the '(new transport)' screen misses the input boxes to specify the Network ID and the Transport ID. So I need to edit the database by hand.

Without the netid & tid the channel scanner doesn't think the transport has any channels, and asks to remove it at the end of a scan.

I also came across this myself using DVB-S. In my case the channels were added but trying to view them resulted in only a partial lock. Manually adding the Network ID and the Transport ID to the DB fixed things. Possibly references #7546.
